Blow-out caused death crash

The death of a 90-year-old woman following a blow-out on the M62 has been recorded as an accidental death by Calderdale Coroner’s Court.

Pauline Ackstein, formerly of Devonshire Court, Salford, died on May 29, 2013, when a tyre on the van she was a passenger in blew out while travelling eastbound between junctions 24 and 25. .

William Adam, of Wellington Street East, Salford, lost control of his Vauxhall Vivaro and swerved across three lanes of traffic and crashed into the barrier, ejecting Mrs Ackstein through a window causing her death.

Keith Reyner, collision investigator at West Yorkshire Police, told the inquest the tyre was bald and that the blow-out was as a result of a bulge in the tyre.

The inquest heard that Mrs Ackstein was not wearing her seat belt.

Coroner Oliver Longstaff said: “If she had been, it is likely she would not have been thrown out of the vehicle.”
